Appointments
Appointments are scheduled Monday through Friday between 0900 and
1500 hours. The length of the appointment varies with the nature of
the problem as well as whether you are being seen for the first time
or for a follow-up visit. We encourage everyone to bring a complete
list of their current medications to the visit as well as a copy of
their insurance card(s).
If you are a new patient, you will be asked to fill out a patient
questionaire. Click

available). Please fill this out and bring to your appointment. We also encourage you to show up 15 minutes prior to your
appointment to allow extra time for your registration.
